This is a limitation of how Unicode was originally designed. The "Enclosed Alphanumeric" block was created for list-making, so it only includes standard letters (A-Z) and numbers (0-20). If you type special punctuation marks, commas, or very rare foreign letters, the generator has no circled equivalent to swap them with, so it outputs the standard character instead.
Unfortunately, yes. Screen reading software (used for visual accessibility) reads exactly what the Unicode code tells it. So, instead of reading the word "Cat", it will read "Circled Latin Small Letter C, Circled Latin Small Letter A, Circled Latin Small Letter T."
